The most important step in relieving the effects of burnout is to identify if there is a problem. Think about the last 6 to 12 months of your life – including work, personal, leisure and family life – and rate the amount of change you’ve noticed.

Each answer represents a numerical value: 1 = little to no change; 2 = small amount of change; 3 = noticeable amount of change; 4 = fair amount of change; 5 = significant change
